Grooms Must Confirm Eligibility by October 14th For COSA Benefits Program

All grooms covered under the COSA Life/Health/Dental Benefits programs are reminded that Annual Eligibility Check Forms must be received in the COSA office by October 14, 2011.

Proof of fulltime employment as a caretaker is required as well, so please attach a copy of your most recent paycheque or a pay stub.

Failure to return the proper paperwork by October 14th will, unfortunately, result in immediate termination of benefits.

Forms are now in the mail, but are also available for download on our website here.

If you have any questions, please contact Cathy Boughton or Stacey Newman at the COSA office

905-854-2672.
