New E-claim Service For COSA Health Plan

 

As of October 1st, COSA members who are enrolled in our group Health Insurance Plan will be able
to submit their own claims online.

Most claims which have not already been submitted electronically by the dentist or pharmacist can now be
entered online through Industrial Alliance’s Cyberclient.

The new service will eliminate the drudgery of paper claims and enable members to check the status of their claims online at any time.

Grooms Must Confirm Eligibility by October 14th For COSA Benefits Program

 

All grooms covered under the COSA Life/Health/Dental Benefits programs are reminded that Annual Eligibility Check Forms must be received in the COSA office by October 14, 2011.

Proof of fulltime employment as a caretaker is required as well, so please attach a copy of your most recent paycheque or a pay stub.

Failure to return the proper paperwork by October 14th will, unfortunately, result in immediate termination of benefits.

Correction Re Healthcare Coverage

To All COSA Members:

In the COSA brochures that were sent out to all members, please note that under
the COSA LIFE-HEALTH DENTAL section that healthcare including prescriptions
drugs, paramedical services, private duty nurse, ambulance are all covered at
80% not 100% as listed in the brochure.

Also, the eye glass coverage is
available to full tme caretakers only.
